OHS6140 - Systems Safety Assessment Answer
Task:

LEARNING OUTCOMES
On completion of this unit, students should be able to:
1. Apply contemporary System Safety theory and practices to initiate policies, and procedures within the workplace.
2. Evaluate and apply System Safety analysis techniques to develop effective life cycle risk management from concept and design through to decommissioning of a plant or process.
3. Employ System Safety and Safety Case principles for the risk management of an on-shore or off-shore Major Hazard Facility.

Assessment 1 – Systems Safety Review - OHS6140
Task:

Review the literature on Systems Safety (using peer reviewed articles and books). Write a review of the literature addressing the following elements:
1. Define Systems Safety.
2. Explain the theory/concept of, Systems Safety.
3. Describe the life cycle or "cradle-to-grave" approach embraced by Systems Safety - from conceptual design through to the decommissioning of a plant or facility.
4. Explain the relationship between engineering risk and reliability.
5. Explain the association between Systems Safety and Process Safety.
6. Discuss how Systems Safety is relevant when managing Occupational Health and Safety for an off-shore process or major hazard facility.
7. Identify the Systems Safety measures that employers should implement in an attempt to comply with legislation and Due Diligence. Demonstrate how these measures are applied in the workplace.
8. Discuss what a “Safety Case Approach” is, and how it demonstrates safety of a system, and assists an employer in establishing a Due Diligence.

Assessment 2 – Quantitative Risk Assessment Project

Task 1 - OHS6140
What is a HAZOP Assessment?
Your assessment should include the following:
• Introduction. Describe in detail the purpose of a HAZOP.
• Body of Assignment: Should include as a minimum:

  •  What type of information is required for conducting a HAZOP?
  •  Clarify the purpose of the HAZOP guide words.
  •  A HAZOP can be applied during any stage of the life cycle of a process or plant development, however, at which two stages do you believe a HAZOP would be most effective?
  •  Emphasise the roles and responsibilities of the HAZOP team.
  •  What is the role of the HAZOP Leader/Facilitator?
  •  It is important to have a team or committee approach to conduct HAZOP analysis. Why this is important especially the team selection.
  •  Provide a concise overview of the HAZOP methodology and the critical steps involved including how the results are integrated into a business.
  •  It is important to record the information generated through a HAZOP. What detail is normally required for a HAZOP study output?

Task 2 -OHS6140
HAZOP Application
Perform a HAZOP on the operation of a coffee pod machine. The HAZOP study of a coffee pod machine should include:
a) The scope is – a design objective “hot fresh brewed coffee in a cup”.
b) Identify the study nodes
c) Conduct the HAZOP
d) Complete a HAZOP Report Sheet such as those suggested by Bahr (2015, p.195).
