Jim Gray, 28 years old, severe depression following suicide attempt Mr. Jim Gray is a 28-year-old single male admitted to the inpatient city Mental Health unit 5 days after a failed suicide attempt and has been diagnosed with major depressive disorder’. Jim is the son of a grazier from a rural community 3 hour north of Brisbane. The farm has been severely affected by the longstanding drought conditions in the district creating a financial strain on the family and Jim is expected to take over the family farm when his Dad is no longer able to work. You are the nurse assigned to Jim’s care for the afternoon shift. At handover, you were informed Jim did not get up for breakfast again, went to lunch reluctantly only because he was compelled to but ate almost nothing, and returned to his bed immediately afterwards. The occupational the therapist reported in the progress notes Jim was still choosing not to take part in any activities, including small group games or one-on-one activity.

Vital Signs:

  •  Blood pressure 125/75
  • Temperature 36.3 C
  • Pulse 66/bpm
  •  Respirations 18/min

Jim has a rope burn mark on his neck caused by the breaking of the rope with which he attempted to hang himself and some minor bruising and broken skin on his arms and legs from the subsequent fall. He was commenced on Venlafaxine and a multivitamin on admission. When you go to introduce yourself to Jim you find him lying on his bed with the covers pulled up high. He appears reluctant to engage in conversation with you. When you address him to introduce yourself, he grunts then mumbles I just want to die and turns over to face the wall away from you.
