The assignment question is: 

As a Registered Nurse, there will be times when your own ideas and experiences of health and illness may challenge your ability to practice Person Centred Care (PCC). Reflecting on these perceptions is a skill which is crucial to implementing PCC. 

Using the Gibbs Reflective Cycle as a framework, submit a reflective essay that describes and analyses one experience of applying the concepts of PCC to the NRSG 138 tutorial case studies. This essay should demonstrate your developing understanding of PCC. 

Case study

Jim Laverty aged 65 is a farmer who owns and manages a sheep property, which is a one-hour drive to the nearest town (population 1100). 

Jim is the primary carer for his mother, who is 80 years old and has early-onset dementia. Jim announces “I need to get back home straight away to make sure everything is OK.” 

Jim is single and lives in an isolated region. He has significant work and carer responsibilities. The current drought means an increased workload and worry about meeting financial commitments. 

Jim is the captain of the local CFA volunteers but finds it difficult to have regular social interaction outside monthly meetings because of his commitments to the farm and caring for his mother. 

Jim: Consider the patient situation ISBAR 

At afternoon handover Nurse Kate gives the oncoming staff the following report: Identification: Hello Everyone, Kate Ryan here - I have been looking after beds 1 to 4. 

Situation: Bed 1 is Mr. Jim Laverty a 65-year-old man who presented to Outpatient Clinic following a fall from a building. Jim lives on a property near Blackall with his 80-year-old mother. His father died in a farming accident twenty years ago 

Background: Jim was unable to get assistance and was in the paddock for several hours before being found by a neighbour. Jim is complaining of pain in right knee & right wrist, he has a minor laceration to right hand. Past history: osteo-arthritis, cholecystectomy (1988), removal of melanoma (2009). Jim smoke 20 cigarettes a day. He is unable to weight bear on his right leg and needs crutches. Has had a total of 10mgs of Morphine for pain and now is feeling nauseous needs an order for an anti-emetic. 

Assessment: Jim is afebrile, has mild tachycardia 105, hypertensive at 150/90, is complaining of pain on weight bearing and extension. Currently in radiology department. 

Recommendation: Needs IV morphine for pain & and order for an antiemetic. Query arthroscopy on Friday. Monitor BP. Assess pain 30mins after analgesia.
