Subject Code : MIST705
Assignment Task:

In this project, students form teams to design and use a small database. Each team selects one business case (given below) and develops a relational database using Microsoft Access. The deliverable will include a project report, the database, and an oral presentation. The project report and presentation should both include:

a) a systematic analysis for the project,

b) an entity-relationship diagram of the database and

c) implementation in Microsoft Access (including the creation of tables, relationships between the tables, queries, and reports).  In the presentation, each team also needs to have a demo of using the database with several queries and reports.

The following is a list of cases for your project.

Case 1: Patient Electronic Medical Record Database
In this case, you will design a relational database for a small medical group. This medical office has increasing workload recently and decides to seek your help to develop electronic medical records.  Your assignment is to build a database to support primary care physicians to track patients.  You start the task with system analysis and an entity-relationship design (the ER diagram) for the database. After your database design (include building the entity relationships) is completed, you will create database tables and populate them with data. Your database should be able to provide primary care physician information including doctor’s ID, name, contact information; patient information such as patient ID, name, address, phone, date of birth, insurance, and their diseases; you also need to collect the disease information including disease ID, disease name, etc.
Your database should be able to generate various queries such as:
•Display patient record by specified patient name
•List all patients with selected doctor
•Find out which disease has the highest occurrence
In addition, your database should also allow you to create various reports. For example, you may create a list of patients grouped by diseases for each primary care physician.

Case 2: Musical Instrument Rental Tracking Database (MIST705)
In this case, you will design a relational database for a musical instrument rental store. This small business provides musical instruments such as violin, cello, viola, flute, bass, drum …, to students at local schools. Due to the increasing demand, the store owner decides to seek your help to track store inventory, rental transactions and create management reports in support of its daily operation. You start the task with system analysis and an entity-relationship design (the ER diagram) for the database. After your database design is completed, you will create database tables, link the normalized tables with relationships and populate them with data. Your database should be able to provide customer information such as student name, address, school, grade, phone, parent name, email; rental transaction information such as instrument serial number, rental start date, due date, deposit, rental fee, fine; store inventory information such as instrument, instrument serial number, current status, purchase date, purchase cost, etc.  Your database should be able to generate various queries such as:
•Display musical instrument rental transaction information by specified customer name
•List customers who are currently renting multiple instruments
•A show which instrument has the highest demand
In addition, your database should also allow you to create various reports. For example, you may create a report displaying all rental instruments due in the next 30 days, along with student name, a grade for each school.

Case 3: Class Registration Database
In this case, you will design a relational database for a local business that provides tutoring and test preparation classes such as SAT, GRE, GMAT, etc. The owner of the business has been using the old fashion pencil and notepad tracking for many years. Due to the increasing demand, he needs a database to help manage its business operation to provide better services. You are taking this challenge and start the task with system analysis, and an entity-relationship design (the ER diagram) for the database. After your database design is completed, you will create database tables, link the normalized tables with relationships and populate them with data. Your database should be able to provide class information such as class number, class title, subject area, instructor ID, and class schedule; instructor information such as instructor ID, name and contact information; student information such as ID, name, address, school, date of birth, phone, email; class enrollment information such as who enrolls what classes, and enrollment date. Your database should be able to generate various queries such as:
•Display class enrollment detail by student name
•List all students who are taking multiple classes
•Find out which classes have the highest demand
In addition, your database should also allow you to create various reports. For example, you may create a report displaying the student contact information by each class for each instructor at the school.

Case 4: Home Theater Sales Database
In this case, you will design a relational database for a home theater sales and installation business. This local home theater specialty business has experienced dramatic growth when they introduce video conferencing system installation for business customers.  In the meantime, the owner decides that he needs a database to manage information of customers, transactions, products and work orders. You are hired to take this challenge, starting the project with system analysis, and an entity-relationship design (the ER diagram) for the database. After your database design is completed, you will create database tables, link the normalized tables with relationships and populate them with data.  Your database should be able to provide customer information such as customer ID, name, address, email, phone, credit card number for billing; transaction information such as transaction number, customer ID, product, date of purchase, installation status; product information such as product ID, manufacturer, model, and price; work order information such as work order ID, technician who does the job, transaction number, date installed, work start time and end time for the installation. Your database should be able to generate various queries, including:
•Display customers of a specified manufacturer
•Display the total number of installed systems by each manufacturer
•Show most popular home theater systems (manufacturer, model)
In addition, your database should also allow you to create various reports. For example, you may create a report of compensation for each technician based on the hours they work (assume they all get paid at the rate of $50 per hour)
