Case Study 1 – Acute Coronary Syndrome (ACS)
At 2100, you are half way through a nightshift working for your Local Ambulance Service. You are called to a male patient who is complaining of crushing central chest pain.
After securing scene safety on arrival, you notice a middle age male on the couch clutching at his chest. You introduce yourself and the patient responds and provides his name, John.
John appears alert and orientated to time and place and reports 9/10 central crushing chest pain.
Your primary survey identifies airway and breathing is clear however, increased rate at 18 breathes/ minute. John has a radial pulse of 120 beats/minute.
On further examination John appears to be severely distressed, looking pale, clammy and diaphoretic. John tells you he is a smoker and had a previous heart attack about 4 years ago.
John’s observations are as follows:
Pulse: 120 beats per minute
Respiratory Rate: 18 breathes / minute
Blood Pressure – 140/90 mmHg
Capillary Refill – 2 seconds
Oxygen Saturations – 97%
Temperature – 37.5
