Task:
Apply throughout the essay the concept of cultural respect in regards to the discussion of Aboriginal and Torres Strait Islander Peoples they will:

1. Provide a summary of what "Closing the Gap" (CTG) policy is. Identify and discuss one (1) relevant CTG target in relation to Judy’s current health problem, using the relevant literature and statistics.

2. Analyze & discuss 3 (three) social determinants, in relation to the potential health benefits and or challenges of Judy being registered with CTG, using the relevant literature and statistics.

3. Discuss the procedure of how Judy was registered with CTG. Define the roles of the Aboriginal Medical Service (AMS) and, Aboriginal Liaison Officer (ALO), and discuss what services they can provide to Judy and the interrelationship between the health systems; such as why did the AMS refer Judy to a mainstream health service for daily dressings.


Task 2:
Framework: You are a
senior RN working at UTS Community Health Centre that does home visits.

Patient Background: 
Your new patient is Judy, a 57yr old Aboriginal woman with type 2 Diabetes who has been discharged home with a leg ulcer for daily dressings. She is a widow, currently living by herself in her own home. Judy has always worked part-time as a primary school teacher but due to her mobility issues has now retired. She does not own a car and relies on public transport. She lives 10km from the closest health care facility. Judy has two grown-up children, who live close by, and who assist when they can. They will be present for the first visit. 

The referral is from the local AMS. The request for daily dressing was from the ALO for the community nurses to attend home visits for daily dressings to the leg ulcer as AMS is not able to attend to this need. The ALO is unable to come to the first home visit for the introduction. 

The accompanying discharge letter from the hospital states that she may be eligible to be registered for CTG
Before attending your first visit, and as part of your preparation for the visit, please answer the following:

1. Write a brief explanation of what "Closing the Gap" (CTG) policy is, including the history and data that influenced the policy's creation. Then discuss the significance of the CTG policy on Aboriginal and Torres Strait Islander People's health outcomes, using the relevant literature and statistics. Please include any challenges and/or barriers that may have affected the changes.

2. Analyze the potential long term consequences of Judy's Diabetes to 3 social determinants of health and, how they are addressed within the CTG policy. Your answer should include a discussion of the CTG policy.

3. Discuss the potential benefits and impact of being registered with CTG on Judy’s social determinants of health. Please discuss this in relation to the 3 social determinants of health discussed in question 2. 

4. Define the roles for the Aboriginal Medical Service (AMS) and, Aboriginal Liaison Officer (ALO). Explain what services they provide.

5. Give the reasons why the AMS and ALO would be involved in Judy’s care and would be referring Judy to the community health center. Describe the relationship between the AMS, ALO and community health center.
