AMIR

Amir is a 3 year old male. He was born in Australia to Syrian parents. English is their second language and while Amir’s parents can participate well in normal conversation, they are  finding difficult to understand medical terminology. Amir is currently up to date with all immunisations. He has a history of middle ear infections, having had 8 in the last 12 months. You are working on the day surgery unit at the local hospital. Amir has returned to the unit after having grommets inserted under general anaesthetic. The ENT surgeon has provided the following post-operative orders:

• For discharge this PM;
• Please provide parents with appropriate discharge information.

Amir is currently sitting up in his bed talking to his mother and father in Arabic. He has an intravenous cannula insitu in his left hand and has had a sandwich and glass of orange juice. He is due for oral pain relief prior to discharge.

 

CLAUDIA

Claudia is a 16 year old female. She is up to date with all immunisations. Claudia lives at home with her mother, father and 2 younger siblings. She is about to start year 11 at college.

You are working in the emergency department at the Royal Hobart Hospital (RHH) on New Year’s Eve. Claudia has been brought in by ambulance from the Falls Festival. She was crowd surfing when she ‘decided to fly’ and jumped from her friend’s shoulders, injuring her right ankle. The paramedics report Claudia was animated and talking excitedly to her friends and had dilated pupils during when they assessed her at the festival. Claudia was given a hand- held Penthrox Inhaler for pain during the ambulance ride. Claudia’s mother has arrived at the ED and is in the cubicle with Claudia. Claudia has been diagnosed with a displaced trimalleolar ankle fracture and has been consented for theatre. She has the following nursing instructions:

• Prepare patient for theatre;
• IVC to be inserted;
• Pain relief PRN;
• Requires a full drug and alcohol history.

 

JESSICA

Jessica is a 10 year old female. Jessica lives at home with her mother, father and younger brother. Jessica was diagnosed with cystic fibrosis during her newborn screening test. She had a number of hospitalisations as a young child, but is now generally well and enjoys playing soccer with her local club. She is up to date with all immunisations. You are working in the emergency department of a large metropolitan hospital. Jessica has been brought in by her parents with a three day history of increased coughing, fever and generalised aches. Jessica has an Infusaport in situ – her father applied Emla on her Infusaport site prior to bringing her to the ED.
 

Jessica has been medically reviewed. She requires:

• IV access;
• Humidified O2 via NP;
• Commence IV antibiotics.
