CASE STUDY 1 - ROSE

Mrs Rose Manning is an 82-year-old lady. 

She has been living in a low care dementia facility for two weeks. 

She has been diagnosed with moderate Alzheimer’s disease and has been coping quite well up until now with the help of her 85-year-old husband.  

She has a great deal of trouble communicating verbally, needing prompting with assistance, sometimes for showering, dressing and grooming.  

Her husband has had a bad fall and feels he can no longer care for her. He is most upset by this situation. She is settling into the facility quite well and her husband visits every day, usually in the afternoon. Rose also has four children who visit often. 

Rose worked as a pastry chef most of her adult life and in later years travelled extensively through Australia and overseas. Rose loved to dance and was always very social. 

Her room is filled with photos of friends, family and many of her travelling highlights. 

She has arthritis and needs assistance when walking; she uses a walking stick but will not walk unless accompanied. 

When Mr Manning visits, he has afternoon tea with his wife at about 1500hrs and has a gentle walk around the garden leaving at 1730hrs. Rose enjoys this very much along with her Jewish religion, enjoying all aspects of her religion and culture. 

You are working on the afternoon/evening shift. Mr Manning telephones at 1400hrs to say he is not feeling well and cannot visit.  

You tell Rose this and she shrugs and walks off. 

At 15:30 Rose becomes distressed, wringing her hands, pacing, crying and looking out of the window.

This behaviour from Rose has caused a fellow resident Beth to start telling Rose to sit down and stop crying.

 

CASE STUDY 2 – MARTIN

Martin is 72 years old and has been diagnosed with dementia resulting in short term memory loss, confusion and recurring suspicions that “they are stealing my money”. 

Martin has been a patient of the hospital for 10 days being treated for an unhealed ulcer. 

He is quite aggressive when approached for routine care such as making his bed or showering. 

The worker approaches him one morning to transport him to the specialist medical clinic for a final check and a new outpatient appointment. 

Martin’s belongings have been packed up for him while he was in the shower and this has confused and infuriated him. 

He is convinced that his money and other precious items have been stolen and as the worker approaches, he begins screaming abuse and lashing out with his walking stick. Martin hits a fellow patient  (Sam) as he is walking past. Sam yells at Martin.

 

CASE STUDY 3 – JOHN

You are working as a community aged care worker for a privately-run organisation with a large number of older clients living at home. Today you have been allocated John as one of your community clients and you are to transport John to a Day Therapy Centre to give his wife Vera some respite from her caring role. John is an 88-year-old retired pharmacist who was diagnosed with Vascular Dementia over a year ago and you have not cared for John before.

When you arrive at their home and Vera opens the door you notice she is quite upset and Vera tells you that John is refusing to change out of his pyjamas into his clothes in readiness for this day respite outing.  Vera asks you to come and help her with John as she is finding him quite resistive this morning and she asks for your help to change him. As you walk into the bedroom Vera starts to raise her voice and grabs at John’s Pyjama top and tries to remove it, but John refuses to lift his arms up.  Before you can assist Vera she starts to yell at John, asking him why “he has to make things so hard for her “and John has now started shouting at Vera as well.

Suddenly John tries to break free from Vera and accidentally knocks her glasses from her face and scratches the side of her cheek. Vera bursts into tears and sits down on the bed sobbing that she can longer cope anymore. As you go to comfort Vera, John turns to you and also starts to cry saying how sorry he is and he didn’t mean to hurt Vera.

 

CASE STUDY 4 - ELIZA

Eliza is a 79-year-old female; recently she was permanently placed in a low-level hostel and diagnosed with mild dementia.

This had to be initiated because she was being evicted from an independent living unit because of her persisting disruptive behaviours that were upsetting other residents. 

She had a history of disruptive behaviours in previous residences including independent living and hostels.

She also has a history of depression, anxiety, chronic back pain and attention-seeking behaviour, and paranoia.

In summary, her disruptive behaviours include: 

  • Talking loudly about Mary, who sits at her dining table, in a derogatory way while Mary is present

  • She is disruptive in social contexts such as meal times and during activities such as bingo.

  • She does not respond to requests to stop or reasoning and Mary looks ready to rage at the table tonight

  • Eliza responds to one-on-one attention, reassurance, and being made to feel special

  • She displays little insight into her behaviour; in fact, she adamantly denies ever saying anything unkind.

Volunteers have now refused to work with her and she was asked to stay away from bingo and other group activities.
