Mark Brown, aged 58 years, presented to his general practitioner (GP) with a seven- month history of constipation and abdominal discomfort. Recently he has felt more fatigued than usual and had been experiencing lower abdominal pain. In the weeks prior to Mark’s consulting his GP, he had noticed his stools were long and narrow, and small amounts of bright blood on the toilet paper. Mark had ignored many of these signs as he thought that his worsening constipation was related to reduced muscle tone and lack of exercise.

The GP performed a digital rectal examination. On examination, the GP palpated a firm irregular non- capsulated mass in Mark’s rectum. The GP arranged a consultation with a gastrointestinal specialist. The specialist performed a colonoscopy. During the colonoscopy, a biopsy was taken of the large sessile lesion located in the proximal third of Mark’s rectum. Three polyps were also removed from Mark’s colon. The biopsy results confirmed a stage IIA rectal adenocarcinoma.
Mark was admitted to the surgical ward in a tertiary hospital for an abdominoperineal resection and the formation of a sigmoid colostomy.
